Job Description:
*Sunrise Services, Inc. is seeking a Lead Direct Support
Professional for our department of Supported Living!*Come join a

lives of others!*About the Program: *Provide direct care support
and assistance to participants in their daily lives, and document
services given. Such support and assistance is provided in a manner
that helps empower the participant and build independence.*Duties &
Responsibilities: **Personnel Related Activities** Monitor staff
attendance by taking absence reports from staff calling in and
forwarding a written report to the CLF;* Monitor staff attendance
by arranging for fill-ins when someone calls in;* Monitor staff
attendance by reviewing staff sign in/sign out sheets at the
beginning and end of each shift; report any discrepancies to the
CLF.* Assist CLF's in organizing daily activities (e.g. getting
participants to work, school, appointments, etc.)* Serve as a
liaison between CLF's and direct care staff at the site as
requested / directed by the CLF's.* Communicate as needed (daily,
throughout the day, weekly) with the CLF's regarding questions,
issues or problems.* Assist CLFs in providing secondary
orientation/training to staff.* Answer questions, provide support
and direction to direct care staff at the site.* Document any
on-shift performance issues and forward written report to CLF.*
Collect and deliver paperwork such as such as Incident Reports,
Employee Injury Reports, receipts, etc. to CLF's on a daily basis.*
Fill in for staff when needed.*Client Related
Activities**Financial** Collect and deliver receipts for all client
financial transactions and submit them daily or ensure that
receipts are submitted by on-shift staff.*Health and Safety** Pick
up and deliver new medication prescriptions and refills as directed
/ requested by the CLF. Verify medications per Sunrise Services
Policy and Procedure.* Provide medication assistance to
participants when regular staff are not available, including Nurse
Delegation needs, as they arise.* Complete WAC Site Safety
Checklists once a month for each participant.* Inspect
apartment/grounds daily to ensure a safe and healthy environment.*
Report maintenance problems to maintenance and/or the CLF.*Client
Program/Activities** Plan for, coordinate and assist with client
transportation for daily activities (e.g. getting participants to
work, school, appointments, etc.).* Under the direction of the
CLF's, assist with planning and coordinating recreational
activities/outings for participants.*Skills** Be able to
successfully work with and provide support to a number of
individuals, some of whom have challenging medical (follow medical
protocols, Nurse Delegation) and behavioral challenges (follow
PBSPs)* Be willing and able to assist with participant hygiene
needs, (toileting, bathing and dressing) as well as Nurse Delegated
assistance, etc.* Possess and demonstrate strong, positive
interpersonal skills.* Possess and demonstrate strong teamwork
skills.* Possess and demonstrate strong communication and
organizational skills.* Be eligible and approved to drive a Sunrise
Services vehicle.*Requirements: ** *Education: * High school
diploma or equivalent.* *Experience: * One year of experience
working with those who have developmental disabilities.*
*Licensure/Certification: ** Valid Driver's License & acceptable 3
year driving abstract.* First aid / CPR certification.*
Fundamentals of Care or Food handler's card.* NAR certification.*
Therapeutic Options training.* Other trainings as required.* *Other
Considerations: ** At least 21 years of age or in an internship in
an applicable accredited program.* Excellent verbal and written
communication skills, including listening skills.* Skill with
Microsoft Office products including Word, Excel, Access and
Outlook. Ability to learn scheduling and employee-database
software.* Understand and implement boundaries specific to children
living in the home.* Participate in appropriate physical activities
with youth.* Operate basic home appliances.* Lifting up to 30 lbs.*
Vaccination or face masks may be required.*Compensation: ** $20.00
- $26.13 Depending on Experience* Wages are determined by
applicable industry experience, education, and
