Case Manager RN in Olympia, Washington, 98506! The RN Case Manager
is responsible for coordinating comprehensive patient care services
to ensure safe and effective care transitions across the healthcare
continuum. The role focuses on discharge planning, utilization
management, patient advocacy, and collaboration with
interdisciplinary teams to optimize patient outcomes and resource

EMR systems (Epic, Cerner, Meditech, etc.) Ability to multi-task
and work independently in a fast-paced environment BLS may be
