Job Description:

and continuing care in the Physical Therapist (PT) role. In this
unique and rewarding role, you will help patients improve mobility,
manage pain, and maintain independencewhile also supporting comfort
and quality of life for those in hospice. The Physical Therapist is
solely responsible for evaluating and diagnosing a patient,
preparing a treatment plan and therapeutic goals, including
determining number and frequency of visits providing or supervising
all care, and determining appropriate discharge of a patient. Youll
work collaboratively with an interdisciplinary team to ensure that
care is personalized, holistic, and aligned with each patients

provide continuing care through these responsibilitiesEssential
functions of this position include the following.* Perform
comprehensive assessments and develop individualized care plans
based on physical, emotional, and functional needs.* Assess and
evaluate therapeutic, rehabilitative and/or functional status;
participate in the development of the total plan of care.* Assess
for muscle strength, mobility gait, ROM-potential for rehab.*
Determine priority needs for therapy patients.* Initiate Physical
Therapy program and instructs other personnel and/or
family/caregiver members in certain phases of physical therapy with
which they may work with the patient, as well as instructing them
as to the goals of the Physical Therapy program for the patient by
participating in case conferences.* Evaluate the home environment
for safety.* Deliver physical therapy services in patient homes,
focused on restoring function, mobility, and quality of life.*
Educate patients, families, and caregivers on therapy goals,
exercises, and techniques for safety and independence.* Collaborate
closely with nurses, social workers, chaplains, physicians, and
other team members (as needed).* Monitor and document patient
progress, revising care plans as needed; participate in discharge
planning.* May train patient in the use of prosthetic and/or
orthotic devices.* Strictly follows infection control procedures
and safety policies.* Adequately supervise patient care given by
assistive personnel such as Physical Therapy Assistants (PTAs) in
accordance with regulations and company leadership principles.*
Maintain compliance with all federal, state, and organizational
policies, including those of the Washington State Department of
Health.* Participate in staff meetings, training, and quality
improvement initiatives.* Participate in continuing education to
stay informed of trends, practice developments and maintain
licensing requirements.Our requirements and qualifications for
success* Graduate of an accredited Physical Therapy program (DPT or
MPT).* Current Washington State Physical Therapist license in good
standing.* 0-2 years' of experience* Current CPR BLS
certification.* Experience in Home Health and/or Hospice is highly
valued.* Reliable transportation, valid drivers license, and proof
of auto insurance.* Basic proficiency with electronic medical
record systems (EMR).* Compassionate, patient-centered care
mindset.* Strong clinical judgment and ability to make ethical
decisions.* Ability to work independently and collaboratively.*
Excellent communication, prioritization and time management
skills.* Respectful and sensitive to patient cultural, spiritual,
and emotional needs.Physical, environmental, and mental
requirements* The ability to listen to, understand and communicate
information and ideas presented through spoken words and
sentences.* The ability to tell when something is wrong or is
likely to go wrong. It does not involve solving the problem, only
recognizing that there is a problem.* The ability to use your
abdominal and lower back muscles to support part of the body
repeatedly or continuously over time without 'giving out' or
fatiguing.* The ability to lift up to 50 lbs.* The ability to
function in other peoples home setting which can vary widely with
