Summary
The RN functions as a care manager for the HBPC assigned veterans, providing care in veterans' homes and is responsible for:

      Duties include but are not limited to:
      • Responsible and accountable for the provision of quality nursing care to a culturally diverse veteran patient population.
      • Possesses the knowledge and skills to utilize the nursing process in a collaborative, interdisciplinary practice setting to implement health promotion practices, manage acute and chronic illnesses, facilitate attainment of optimal levels of functioning, and to support the dying patient and his significant others.
      • Functions as a care manager linking the patient and the caregiver with the various health services offered by the HBPC team and the Medical Center.
      • Teaching the patient and caregiver effective and efficient ways of managing and delivering nursing care in the home.
      • Monitoring the patient's condition and the care provided in the home.
      • Supervising any nursing students assigned to the HBPC program.
      • Communicate and interact appropriately with all internal and external customers; the registered nurse II collaborates with the Interdisciplinary team to manage and coordinate health care services that the patient receives in the home. Assists with planning and coordinating the patient's treatment plan with all members of the interdisciplinary team.
      • Provides leadership in improving and sustaining the quality and effectiveness of care in diverse or complex patient care issues.
      • Maintain confidentiality of electronic, written, and/or verbal patient/employee information as well as patient communications, protected under the provision of applicable laws and the Privacy Act.
      • Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment.
      • Provide age-specific care for adults of all ages, particularly the older adult and geriatric population which includes the following elements related to the process and/or effects of chronic illness: promote independence; encourage self-care; involve family or designated caregiver with care; plan for necessary safety measures; and assist patient to cope with and/or compensate for changes affecting mobility, balance, cognition, eyesight, hearing, musculoskeletal system, skin, and thermoregulation. Assess age-specific care requirements, cultural, religious, and gender variables and develop appropriate plan of care.
      • Perform a home safety assessment and make recommendations to improve safety in the home.
      • Assess and meet the unique needs of the dying patient and his/her caregiver or significant others.
      • Works effectively within the home setting to provide preventive health care.
      • Ensures that workload reporting mechanisms for data maintain proper integrity by using proper coding/encounters and by charting notes w/in policy guidelines.
      • Participates and/or supports research that promotes the concept of home based primary care and improves patient outcome.
      • Uses scientific methodology to analyze and evaluate the effectiveness of current standards of care and practice. Utilizes findings to identify alternatives to existing clinical practice, as well as to develop and implement a plan for change in patient care and/or practice.
      • May do the initial assessment and is responsible for visits to the home of patients admitted to the home based primary care program. m. Initiates interdisciplinary projects to improve organizational performance.
      • Initiates and/or participates in quality improvement projects within the program.
      • Initiates action to resolve ethical/legal issues related to clinic practice in collaboration with resource personnel.
      • Maintains knowledge of current techniques, trends, and professional issues to enhance own performance through continuing education, review of current health care literature and/or professional affiliations.
      • Actively evaluates own performance and is self-directed in pursuing knowledge and maintaining proficiency in both clinical and administrative theory and practice.
      • Acts as a resource for staff in identifying and supporting opportunities for career development. Assists with scheduling team in-services, developing and coordinating orientation programs. Pay: Competitive salary, regular salary increases, potential for performance awards

        Requirements

          Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
          Basic Requirements:
          • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
          • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
          • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
          • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience:
            • Home Care experience is preferred Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
              • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
              • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
              • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
              • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
              • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
                Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.
