Population Health Clinical Coordinator -Remote Position -at Providence Medical Group Northwest Washington

The Population Health Clinical Coordinator is responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within NWWA Providence Medical Group (PMG). The Population Health Clinical Coordinator assures that an integrated approach to the patients' care coordination is fulfilled. This position is the liaison between PMG providers, primary care clinics, specialist offices, patients, multiple Providence St Joseph Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes.

Required qualifications:

  • Coursework/Training: Completion of education or training that satisfies the requirements for certification with the Washington State Department of Health.

  • Experience with electronic medical records

  • Epic or other Electronic Health Record /care management software experience

  • Excellent typing skills, computer keyboard, and mouse skills for expedited data processing

  • knowledge of medical terminology, disease processes, medical decision making, and prior experience in utilizing disease registries or similar databases

  • excellent verbal communication and interpersonal skills, with preferred experience in motivational interviewing / coaching

  • proficiency in data processing skills such as word processing and email. Knowledge and experience in utilizing databases and spreadsheets

    Preferred qualifications:

    • Coursework/Training: College education

    • Bilingual in either Spanish, Arabic, Russian, Vietnamese

    • Computer skills including Microsoft Windows and Office Applications (Word, Access, and Excel)

    • Medical Assistants - A current valid State Board registration in good standing or Medical Assistant Certification to match education requirements above

    • Certification through a nationally recognized Medical Assistant program, such as AAMA, AMT, RMA, CCMA or NCCT

    • Community Health Worker Certification
