Description Providence is calling a Registered Nurse (RN) Home
Care (Home Health Snohomish, Day Shift, 1.0 FTE) to Providence Home
Services in Snohomish County, Washington. Home Health nurses will

for eligible external candidates who meet all conditions for

day of employment. In this position you will:
- The RN Case Manager will perform comprehensive nursing
assessments, develop plans of care, coordinate care with other
disciplines and provide direct patient care in the implementation
of outcome oriented nursing interventions under the physician
ordered Plan of Care.
- All required documentation will be complete, accurate and
timely. The most common methods involve using Outlook, laptop
and/or PCs, and our EMR (electronic medical record) system(s).
- Promotes and practices patient/family personal and
environmental safety while performing job duties according to
agency policies and procedures.
- Participates in relevant educational activities to provide
competent care ot patients/caregivers, to improve the nursing
process, and to seek personal and professional growth.
- Displays professionalism in the work environment in all areas
including communication with both internal and external
customers.
- Maintains productivity as per agency standard.
- Complies with all applicable Federal, State, and local laws,
regulations, and CHAP requirements as well as facility specific
policies and procedures.
- Maintain confidentiality of all patient demographics, medical
and financial information at all times. Maintain confidentiality of
all company and client information. Ensure proper handling and
disposal of confidential documents and adheres to all HIPAA rules
and regulations.
- Adheres to all policies and procedures including regulatory
requirements with regard to infection control, practicing standard
precautions, sanitation, and safety and disaster preparedness.
Required qualifications for this position include:
- Graduate of a school of nursing.
- Current Washington State Registered Nursing License.
- Current Washington State Driver's license and have access to
daily use of an insured, reliable vehicle.
- Proof of current Auto Insurance.
- Current Healthcare Provider CPR certification. Preferred
qualifications for this position include:
- 2 years full time experience practicing as a registered nurse
in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing
Facility.
- 1 Year Recent, hands-on technical clinical experience
(occurring within the last three years) in a Medicare-certified
Homecare or Hospice agency/facility doing intermittent visits and
case management.
- Recent hands-on experience in wound care, venipuncture, ostomy,
catheters, and feeding tubes.
- Bachelor's Degree About the ministry you will serve. Providence
