The Everett Clinic, part of Optum is seeking an entry level Behavioral Health Case Manager II to join the team in the Psychiatry/Behavioral Health departments. This is a full-time position working 40 hours weekly 8am-5pm.*We are offering a Sign on Bonus in the amount of $8000.*Location: Hybrid working part-time from home and part-time in Everett.The entry level Behavioral Health Case Manager II functions as an integral member of the healthcare team and is responsible for:Under general supervision from a licensed mental health therapist, the Behavioral Health (BH) Case Manager is responsible for coordination of care for therapy and psychiatry patients. The Case Manager performs clinical triage for level of care needed and patient/provider matching for Psychiatry; serves as a bridge between medical providers and behavioral health providers to implement treatment plans; and works with patients to optimize adherence to their treatment plan.The entry level Behavioral Health Case Manager II is responsible for and not limited to:

  • Receive requests for patients to see a psychiatric prescriber. Review clinical information obtained either by chart review or with patient over the phone and determine level of provider needed using a clinical guideline while matching patient provider preference (i.e., location, personality, specific skills, etc.).
  • Review transfer of care requests and assign patients to new prescriber or therapist. Communicates with patient regarding change in providers.
  • Recognize when a caller/patient is in need of crisis intervention and arrange for telephone triage with the on-call clinician or arrange same day appointments with a licensed BH clinician.
  • Assist in Care Coordination work with medical teams, BH clinicians, psychiatry prescribers in care planning using a care plan template and care pathway protocols. Assists in the creation of care plans and documentation/entry into Healthy Planet once available.
  • Follow up calls/welfare checks on patients to ensure patient follow through on treatment plan (i.e., Did patient pick up their medication? Is patient taking medications, etc.
  • Assist patients in finding substance abuse treatment in the community. Follows up with patient to promote follow through.Preferred Experience and Credentials:Knowledge: Knowledge of mental health conditions, diagnoses and treatment standards of care. Understanding of the mental health system in both the public and private sector. Knowledge of community resources and referral agencies. Familiarity with ICD-10 and CPT coding in relation to mental health conditions and services.Skills: Excellent written and oral communication skills. Must have skill in motivational interviewing and coaching, with detailed follow through and ability to contract for safety. Organizes, prioritizes, and coordinates multiple activities and tasks. Problem-solves with creativity and ingenuity.Abilities: Able to balance multiple priorities; thrive in an environment where urgent matters can cause frequent interruptions. Able to interact effectively with people of varied educational, socioeconomic and ethnic backgrounds, skill levels and value systems. Remains calm and effective in high pressure and emergency situations. Ability to adapt to changes in the work environment and to shifts in organizational philosophy and expectations. Works with initiative, energy and effectiveness in a fast-paced environment interacting with leadership and interdisciplinary clinical staff. Ability to write reports and document patients' progress within established timeliness standards.Education: Bachelor's degree in psychology or related field of study; Master's degree in a mental health related field of study preferred.Experience: Minimum of one (1) year experience with Master's degree and three (3) years' experience with Bachelor's degree as a Case Manager or related position; mental health, crisis intervention, and call center experience preferred.Certificate/License: Licensed Mental Health Counselor Associate (LMHCA) preferred. Associate level licensure with Washington State for counseling required.-Full COVID-19 vaccination is an essential requirement of this role.
