At Cocoon House we empower young people, families, and communities in Snohomish County to break the cycle of homelessness through outreach, housing, and prevention. The Connections Host Home Program is community-based model that utilizes a nationally established form of crisis housing for youth between 12-17 years-old. Your role as a Host Home Case Manager (Youth Case Manager) is integral to the program and you can provide impact and input in its growth and success. Your heart and passion for working with youth experiencing homelessness is a key to your success in this position.

The Host Home Case Manager (HHCM) provides case management for youth who have entered the Connections Program. In tandem with the Host Home Program Coordinator, the HHCM will work to create a supportive and welcoming environment for both youth and host home families. Taking the lead in engaging youth in our program and working closely with them to discover goals, learn essential life skills and help them find long-term housing options is the focus of what you will do. As we build this program, your input in how best to serve our youth will be encouraged and welcomed.

Host Home Case Manager Duties & Responsibilities

Full-Time - On-site

Wage: $20-21/hr

Develop, manage, and engage in individualized case plans for youth in the Host Homes Program including:

  • Ongoing meetings and check ins by partnering with youth in Host Homes across Snohomish County and provide trauma informed care and engagement
  • Support reconciliation of youth with their families
  • Facilitate and coordinate communication between clients, family members, providers, and other agencies
  • Make appropriate referrals for medical, dental, health, and substance use needs
  • Mediate, de-escalate and provide crisis intervention with youth and Host Home families
  • Assist clients in accessing services and resources
  • Maintain accurate and up-to-date client records
  • Follow mandated reporter requirements through documentation of incidents and calling CPS appropriately
  • Ensure that clients are following guidelines of the program
  • Collaborate with community partners and Safe Place to identify immediate housing needs for youth
  • Attend community meetings and direct wrap around meeting with staff and youth
  • Transport youth to appointments as needed
  • Collaborate with Host Home Program Coordinator to ensure safe and successful stays for the youth
  • Participate in the on-call rotation that is 24/7 support for youth and Host Homes

      General:
      • High school diploma/GED or high school equivalency
      • 1 year working with youth or young adults experiencing homelessness or similar populations
      • Must be 21 years of age or older.
      • Clear a background check with DCYF.
      • Possess a valid Washington State driver's license, insurance and reliable transportation.
      • Spanish speaking bi-lingual preferred Physical:
        • Ability to effectively communicate with all internal and external clients
        • Be able to hear voices and alarms.
        • Ability to read and comprehend a variety of materials
        • Ability to operate phone, computer, and record information for files and logs
        • Must be able to lift up to 50 pounds
        • Ability to travel for various distances through urban or rural environments in various weather conditions
        • Must have ability to run up and down indoor and outdoor stairwells in case of youth emergency.
