Description:
At Cocoon House we empower young people, families, and communities
in Snohomish County to break the cycle of homelessness through
outreach, housing, and prevention. The Host Home Case Manager
(HHCM) provides case management for youth who have entered the
Connections Program. In tandem with the Host Home Program
Coordinator (HHPC) and the Host Home Specialist (HHS), the HHCM
will work to create a supportive and welcoming environment for both
youth and host home families. Taking the lead in engaging youth in
our program and working closely with them to discover goals, learn
essential life skills and help them find long-term housing options
is the primary focus of what you will do. Your role as a Case
Manager is integral to the program and you can provide impact and
input in its growth and success. This is mobile case management and
you will work throughout Snohomish County. Your heart and passion
for working with youth experiencing homelessness is a key to your
success in this position.
Our Host Home Case Manager.....
Works with community partners and Safe Place to identify immediate
housing needs for youth
Willing/able to work flexible days/hours including some weekends
and evenings based on program needs (Note: This provides some
flexibility for personal schedules as well, within a 40-hour
week
Provides case management and support to youth in Host Homes
providing trauma informed care
Works with the HHS and HHPC to ensure safe and successful stays for
the youth
What does the Job Look like? Days Full-time: 40 hrs/week
Help provide mediation and crisis intervention to youth
Be willing and able to be a part of an on-call rotation for youth
intakes and after hours needs
Support reconciliation of youth with their families
Complete program paperwork and data entry to meet necessary
deadlines
Communicate with youth, host homes, and program coordinator on a
regular basis
Provide Supportive case management for youth in South County

Pay Scale: $21/hr Cocoon House will provide reasonable

Requirements:
General:
1 year of experience working in human services with at least one
year of experience with parents of teens and one year with youth
and young adults.
Spanish speaking/bi-lingual preferred.
Must be 21 years of age or older.
Show proof of a COVID-19 Vaccine.
Clear a background check with DCYF.
Possess a valid Washington State driver's license, insurance and
reliable transportation.
Physical:
Ability to effectively communicate with all internal and external
clients
Be able to hear voices and alarms.
Ability to read and comprehend a variety of materials
Ability to operate phone, computer, and record information for
files and logs
Must be able to lift up to 50 Ibs
